My Psychiatric H&P Interview Template is a one-sheet (back and front) template that is useful during the interview to help guide your questioning, and organize your notes. It's also useful for writing up a note afterwards, using the templates below. It also includes a full MMSE for convenience.
My SOAP Note Templates are simple text files that can be used to scaffold a basic note. This is especially useful for writing notes for classes, for the Student Outreach Clinic, or anywhere that you're unable to use a proper EMR with dot phrases (such as the VA).
My collection of emergency medicine dot phrases amassed over the years.
